Contains glass bottle of "Eye and Nose Drops 1/2 Fl. Oz. (14. 8cc)" with syringe. "FSN 6505-754-2809, LOT JAZL Jan. 1963" Alcon Laboratories, Inc., Fort Worth, Texas

Both the front and the back of box reads, "FSN 6505-754-2809, Eye and Nose Drops, 1/2 Fl. Oz. (14. 8cc), Caution: Federal Law Prohibits Dispensing Without Prescription, Alcon Laboratories, Inc, Fort Worth, Texas"  One side reads, "For irritant smoke casualties, for relief of pain and congestion in eyes and nose. Use 1 or 2 drops in eye or 2 drops in nostril. This dosage should not be exceeded. Apply as often as needed." While the other side reads, "Contains: Tetracaine Hydrochloride, U.S.P., 0.25% and Phenylephrine Hydrochloride, U.S.P., 0.25% in Purified Water, U.S.P. Preserved with Chlorobutanol (chloral derivative) U.S.P., 0.40% and Sodium Bisulfite U.S.P., 0.10%"
